Description:  
A multi disciplinary team of occupitional therapists, physiotherapists, nurses, social workers and technical instructors has been set up under the line management of a coordinator to facilitate earlier discharge, prevent hospital admission and provide patients in the community with intensive short-term rehabilitation (six weeks) to improve their independence and reduce the need for long-term care packages.

Using elderly mobility and Bartel outcome measurements we have made a major impact in patients independence in their own environment and reduced the need for long- term care packages.
